I give my permission for this information to be used to allow a check to be done on my suitability for the ISPs and to be retained for policing purposes (all personal details provided will be deleted 6 months after withdrawal from panel membership). All panel members will be asked to provide a photocopy of photo ID (passport / driving licence) prior to commencement.
