HRA General Feedback Survey

 
Introduction

This form is designed to allow you to provide feedback on a number of aspects of HRA and UK wide processes.

Any data you provide in this survey will be used to inform the future development of HRA processes and may be shared with internal and external stakeholders.  

If you have requested follow up contact on the feedback you have given, we will use the email address you have provided to contact you. The views you have expressed will not affect the outcome of any application for HRA Approval you have made or might make in the future.
If you have provided your name or any other information that could lead to your identification either as an individual or an organisation, we will remove this before external publication.

You have the right to withdraw your consent for feedback data to be used by the HRA. If you wish to withdraw your consent, please email quality.assurance@hra.nhs.uk to advise that you wish to withdraw consent and all feedback data (identified as being provided by yourself) will be deleted.

Accessible versions of this survey are available upon request.
