The information collected in this survey will be used to inform the future development of the amendment tool and help us to improve the service that we offer.  The information you provide may be shared with our partner organisations in the Devolved Administrations (Healthcare Research Wales, Health and Social Care in Northern Ireland and NHS Research, Scotland) as these changes to the amendment process were implemented UK-wide as part of the Four Nations Compatibility Programme.

Any comments you make will not be published in an attributable manner without your explicit agreement.
